* As a Caring and Sharing Exchange (CSE) hamper sponsor, I understand and agree to the following: My role is to prepare and deliver the assigned hampers as part of the Christmas Exchange program. I will use client information only for the purpose of preparing and delivering the assigned hamper(s). I agree not to share, copy, or store any client information outside of what is required for this program. I will take appropriate steps to protect any personal information provided such as keeping printed lists in a safe place or ensuring digital files are password-protected. Once deliveries are complete, I will permanently delete or securely destroy all client information (for example, by shredding paper copies or deleting emails). If I suspect that any client information has been lost, shared, or accessed by an unauthorized person, I will inform the Caring and Sharing Exchange immediately.
